From time to time Allsortz Youth Club will want to take photographs of activities at youth club, in order for your child to appear in any official photographs taken for use by the club we require the written permission of the parent/guardian.

If you are happy for your child to be included in these photographs Please ensure you fill out a consent form and return it to a member of the committee as soon as possible as we do not wish to photograph your child unless you are happy for us to do so. 

By signing the form you are agreeing to your child being photographed for the purpose of promotional material including website gallery and newspaper photography.
